Description of Project:
|
The project aims at establishing a translational network among the partner organisations in order to promote the development of the concept of integrated health and social care for the most vulnerable people (with special regard to disabled, elderly people, minors at risks, drug-addicted, migrants and ethnic minorities, etc) by:
- confronting "models of good practices" vs critical arguments
- sharing views of national definition and approaches to health and social care integration
- identifying and analysing structural, organisational, economic and social-cultural factors and actors that constitute integrated and sustainable health care and social assistance systems for the most vulnerable groups
- comparing and evaluating different modes of social services delivery.
Project Objectives:
The key specific objectives are the following:
- to support national and local welfare reforms
- to improve the capacity of national and regional public institutions to plan, implement and manage integrated health and social care policies effectively identifying vulnerable groups, main management tools for the health and social care services negotiated planning
- to assess models of integrated care with respect to their efficiency, quality and user-acceptability
- to design service delivery mechanisms that effectively deliver assistance to vulnerable groups and involve the private sector, civil society organisations and community organisations where it is most cost effective to do so also developing models for the contracting out of social services.
- to improve processes and opportunities that enable vulnerable groups to participate in identifying their needs and design responses to those needs
- to develop performance indicators for use in evidence based policy making
- to contribute to the development of research methods for evaluating successful integration of health and care systems etc

Services Provided:
|
- Setting up thematic groups with the role of preparing the discussion papers and materials for further organisation of Conferences
- 11 transnational conferences will be organised in all the partner's country ( Romania will include 2 conferences)
- 5 exchange programmes will be organised in Italy, France, Spain, France and Finland addressed to public and private key actors from the countries involved in the project
- Reports which will contain an overview of exchange of views at conferences
- 1 multimedia Handbook (on DVD) on best practices and experiences developed in Italy, Greece, Spain, France and Finland
- Project's web site promotion and communication materials
- 1 Newsletter (no 10 editions) presenting the project's activities information on EU Call for proposals and the main EU and national initiatives on social inclusion and also other relevant information
- Press releases about different events developed under the projects, articles to be presented in scientific media about social inclusion aspects
- Monitoring and evaluation reports
